1. Original School Report Card (now known as SF9)

2. PSA Birth Certificate

3. Certificate of Good Moral Character

4. 2 pcs 2x2 picture

STEP-BY-STEP PROCESS FOR ENROLLMENT

ONLINE

1.      Log in to Edustria student portal student.edustria.edu.ph

2.      Click “Upload Requirements” to upload the following admission requirements:

• Form 138

• 2x2 ID Picture

• Birth Certificate

• Scholarship Requirements*

3.      Wait for your requirements to be verified. Once your documents have been verified, you will be assigned to a section.

4.      Click on assessment from the menu and confirm your preferred payment scheme.

5.      Pay through Unionbank, or BPU. Then, upload a picture of your proof of payment.

6.      Wait for you timeline to show that your payment has been verified.

7.      Print your certificate of enrollment by clicking on “Print Certificate of Registration” from the Menu.
